LillyDirect self-pay: Zepbound vials, KwikPen tiers and the 45-day refill rule

LillyDirect is Lilly's own pharmacy channel. For Zepbound it is where the self-pay prices live, and it is the reason a cash-paying patient in 2026 can buy an FDA-approved tirzepatide product for a third of the list price. This page is about how the program works; the Zepbound price page has the full price card.

The tiers as of September 4, 2026

The same figures apply to a one-month single-patient-use KwikPen (one pen, four doses) and to four single-dose vials. The program price for 7.5 mg and above holds only if each refill is bought within 45 days of the previous fill's delivery; miss it and the regular price applies until you re-qualify. Lilly's HCP page dates the current program from February 23, 2026.

What is and is not included

Included: the medication and home delivery from LillyDirect's pharmacy. Not included: the prescription. You need one from any licensed prescriber. Lilly's site lists independent telehealth providers for people who do not have one; their visit fees are theirs, not Lilly's, and they vary. Not included with vials: syringes and alcohol swabs, which a pharmacy or retailer sells separately. Not included with the KwikPen: pen needles. How the program got here

• August 2024: Lilly launched Zepbound single-dose vials in 2.5 mg and 5 mg through LillyDirect at $399 and $549 for a four-week supply, the first self-pay route below list.
• February 25, 2025: vials moved to $349 (2.5 mg) and $499 (5 mg), 7.5 mg and 10 mg vials were added at $599 and $699 regular, or $499 within a 45-day refill window.
• November 6, 2025: the agreement with the U.S. government set the multi-dose pen at $299 for the lowest strength and up to $449 for the others, described by Lilly as a $50 discount to the then-current direct-to-patient prices, and made Zepbound a TrumpRx product.
• December 1, 2025: vials followed, to $299 to $449 by strength.
• February 23, 2026: the current Self Pay Journey Program terms took effect per Lilly's HCP page.

Each step lowered the price at the maintenance strengths more than at the starting strength; the 2.5 mg vial fell from $399 to $299 over the period while the 10 mg fell from $699 to $449.

Foundayo and Mounjaro on LillyDirect

Foundayo uses the same structure with a 30-day month: $149 (0.8 mg), $199 (2.5 mg), $299 (5.5 and 9 mg) and $349 (14.5 and 17.2 mg) regular, with $299 at the top two strengths within a 45-day refill window. Reading a LillyDirect quote

• A one-month fill is 28 days for injectables. $449 every 28 days is $5,853 a year, which is $488 per calendar month.
• Titration means three or four different prices in the first four months. On the label schedule (2.5 mg for four weeks, then 5 mg, then steps of 2.5 mg no sooner than every four weeks) a self-pay patient pays $299, $399, $449 and $449 in months one to four if each refill lands inside the window.
• The KwikPen and the vials cost the same, but only the KwikPen is in the Medicare GLP-1 Bridge, which matters if you or a family member is on Part D. See the Medicare page.
