Zepbound price: list, self-pay tiers, savings card and Medicare, dated

Zepbound is tirzepatide approved for chronic weight management and for obstructive sleep apnea with obesity. It has one list price, a self-pay ladder that steps with the strength, a commercial savings card with two caps, and, since July 2026, a flat Medicare price for one of its three presentations. Each is below with the page it came from.

List price

$1,088 per month, as quoted in the February 6, 2026 White House release announcing TrumpRx.gov. Lilly's own pricing page for Zepbound renders its figure in the browser and could not be captured as text on September 4, 2026; the government release is the most recent dated public statement of the monthly list figure. Self-pay through LillyDirect

Lilly's savings page, read September 4, 2026, states one-month self-pay prices for the single-patient-use KwikPen or four single-dose vials:

The $449 figure is a purchase offer, Lilly's Self Pay Journey Program, and it holds only if the refill is bought within 45 days of the previous fill's delivery. Miss the window and the regular column applies until you re-qualify. The HCP page dates the current program from February 23, 2026.

For context on how fast this ladder has moved: Lilly's February 25, 2025 release priced the vials at $349 (2.5 mg), $499 (5 mg) and $499 for 7.5 and 10 mg within the refill window, with $599 and $699 regular. The November 6, 2025 agreement with the U.S. government brought the multi-dose pen to $299 at the lowest strength and up to $449 at higher ones, and the vial followed on December 1, 2025. Savings card with commercial insurance

If a commercial plan covers Zepbound, the Lilly card lets you pay as little as $25 for a 1-, 2- or 3-month fill. The card pays at most $100 per one-month fill ($200 per two-month, $300 per three-month) and at most $1,300 per calendar year, and it expires December 31, 2026. Medicare, Medicaid and other government beneficiaries are excluded. If your copay is $150, the card takes it to $50, not $25; if your plan does not cover Zepbound at all, the card does nothing and the self-pay ladder is the fallback. Medicare

Zepbound is in the Medicare GLP-1 Bridge: eligible Part D enrollees pay $50 per month from July 1, 2026 to December 31, 2027. Two constraints matter. CMS covers only the Zepbound KwikPen; the single-dose vial and single-dose pen are excluded, and pen needles are not included. And the Bridge is for weight management under CMS clinical criteria (BMI 35 or more; BMI 30 or more with heart failure, uncontrolled hypertension or chronic kidney disease; BMI 27 or more with pre-diabetes, prior heart attack, prior stroke or symptomatic peripheral artery disease), not for a sleep apnea prescription, which goes through regular Part D. What a month really costs

A 28-day fill is 13.04 fills a year. At $449 that is $5,853 a year, or $488 per calendar month, not $449. Run your own strength and program through the calculator above; it shows the arithmetic and lets you add a telehealth membership or pen needles on top.

Compounded tirzepatide is a different product

Compounded tirzepatide vials are priced by the pharmacy or telehealth provider, not by a list price, and they are not FDA approved or interchangeable with Zepbound.
