Ozempic price: list per pen, NovoCare self-pay by strength, and the 48-month card

Ozempic is semaglutide approved for type 2 diabetes, cardiovascular risk reduction in type 2 diabetes, and kidney outcomes in type 2 diabetes with chronic kidney disease. It is not approved for weight management; that is Wegovy. The price structure is Novo Nordisk's, so it rhymes with Wegovy's, with one difference at the top strength.

List price

$1,027.51 per pen, per NovoCare's list-price page read September 4, 2026. An Ozempic pen holds four weekly doses at its labelled strength (the 0.25/0.5 mg pen is 1.5 mL, the 1 mg and 2 mg pens are 3 mL), so one pen is a 28-day fill. TrumpRx.gov shows the same $1,027.51 as the original price. The list price falls to $675 per month on January 1, 2027.

Self-pay through NovoCare Pharmacy

The November 17, 2025 release set these prices and noted that they are available through retail pharmacies, NovoCare home delivery and telehealth partners as well as NovoCare itself. Unlike Wegovy, Ozempic's top strength costs more: $499 for the 2 mg pen against $349 for 1 mg. Per milligram it is the other way round, which is the usual pattern and the reason the price per mg page exists.

Savings card with commercial insurance

With a commercial plan that covers Ozempic: as little as $25 per month, card contribution up to $100 per month, usable for up to 48 months. One month is one box of one pen. Government beneficiaries are excluded. The 48-month duration is a feature the Lilly cards do not have; the Lilly cards instead carry a calendar-year cap and an expiry date. Medicare

Ozempic is not in the Medicare GLP-1 Bridge, which covers only weight-management products. Prescribed for type 2 diabetes it is a standard Part D drug. Semaglutide was selected for the second round of Medicare price negotiation; CMS's negotiated price for the semaglutide products (Ozempic, Wegovy, Rybelsus) takes effect in January 2027 at $274 per month. That is the price Part D plans pay; what a beneficiary pays depends on the plan's cost sharing and the Part D out-of-pocket cap. Ozempic prescribed for weight loss

Ozempic's label is for type 2 diabetes. When it is prescribed off-label for weight loss, insurance usually does not cover it, the savings card terms require use for the approved indication, and the self-pay ladder above is what applies.
